CNA - LTC/SNF

Job Summary:
Provides essential supportive services to residents under the supervision of nursing staff, assisting with activities of daily living and ensuring comfort and safety. Supports resident mobility, nutrition, hygiene, and documentation to promote optimal care.

Responsibilities:

• Assist residents with activities of daily living, including lifting, turning, positioning, and transferring them into and out of beds, chairs, and wheelchairs.
• Coordinate dining services, deliver meals and nutritional supplements, and assist residents with feeding as needed.
• Document meal intake, weight changes, and resident condition accurately and promptly.
• Monitor skin condition, report pressure areas and skin breakdowns, and support wound prevention efforts.
• Support toileting programs, complete flow sheets, and update nursing staff on resident condition changes.
• Report any factors interfering with care delivery, such as resident refusals or reassignment needs.

Qualifications:

• Minimum of 1 year experience in long-term care (LTC) required.
• High school diploma or equivalent required.
• Valid BLS certification through AHA required.
• Active state license as a CNA required at time of submission.
• Proven work history with employment lasting longer than 13 weeks at previous roles.
• Successful completion of skills checklist within the past year and two supervisory references from the last two years.
