Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments to identify mental health, substance abuse, and medical needs in a primary care setting.
\n
• Deliver individualized behavioral health services through counseling, intake interviews, diagnosis, treatment planning, and crisis intervention for diverse client populations.
\n
• Perform thorough documentation of mental health evaluations, progress notes, and group counseling sessions using our electronic health record system.
\n
• Conduct suicide risk assessments and implement appropriate interventions and care plans.
\n
• Serve as a liaison between clients and external community resources, offering referrals to support local services, including medication assistance programs and substance use disorder treatment.
\n
• Collaborate with our Quality Improvement teams to improve care processes and contribute to data collection initiatives that enhance service delivery.

Patient Population / Client Focus\n

In this role, you will work with a diverse client base ranging from children to older adults, addressing an array of mental health concerns such as anxiety, depression, trauma, and substance abuse. You will employ culturally sensitive practices to ensure that all clients receive appropriate care tailored to their unique backgrounds and life experiences.

Qualifications\n

• Must hold a valid license as a Licensed Clinical Social Worker (LCSW) in the state of New York.

• Possess a Master’s degree in Social Work from an accredited institution.

• A minimum of three (3) years of post-licensure experience providing mental health services.

• Demonstrated knowledge of counseling techniques, crisis intervention strategies, and the ability to make independent judgments when delivering direct services.

Compensation and Benefits\n
