Social Worker

Overview At Centerville Day Treatment, the Clinical Mental Health Professional utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care, assess crisis situations, and provide family and individual treatment. The role works collaboratively with patients, families, healthcare providers, insurers, community resources, and other involved parties to deliver care appropriate to the established plan of care in the assigned patient population. Job Specifics Pay Range: $35.25 - $54.39 (exempt) Benefits Eligible: Yes FTE: Full time Shift: Monday - Friday, 8:00am – 5:00pm Required Licensure: LCSW, CMHC, or LMFT Location: Centerville Day Treatment, Centerville, Utah Essential Functions Diagnoses and treats various mental, emotional, and behavioral disorders, modifying care plans to reflect changes in patient status. Refers patients to other services to improve their well‑being, including community services, treatment programs, support groups, psychiatrists, or other mental health providers. Works with the multi‑disciplinary team to develop a discharge plan to provide further care, services, and referrals to ensure continuity of care. Organizes and prioritizes daily work by assessing new, current, and discharging patient needs within areas of responsibility. Performs behavioral health consultation, conducts psychosocial assessments, and provides supportive, crisis, bereavement and other behavioral health interventions as necessary. Educates, communicates, and coordinates with multiple patient care teams and outside agencies to ensure quality care, timely discharge, and resolution of delays. May provide support and debriefing, in conjunction with the Employee Assistance Program, for staff members following adverse events. Minimum Qualifications Master's Degree in Social Work or a related field from an accredited institution. Current State Licensure as LCSW, CMHC, or LMFT. Excellent written and verbal communication skills. Computer skills (e.g., email, word processing). Preferred Qualifications Experience working with patients, families, and treatment teams. Physical Requirements Ongoing need to see and read information, assess patient needs, operate monitors, and identify equipment. Frequent interactions with patients, staff, and visitors requiring verbal communication and listening in emergency situations. Manual dexterity to manipulate equipment and perform typing for documentation. Ability to lift and transfer patients, carry supplies up to heavy items, and maneuver equipment.
