Physical Therapist (PT) – PRN | Home Health – Camden, TN Area Transform Lives—One Home Visit at a Time Are you a Physical Therapist (PT) looking for a flexible role with purpose and real impact? At Tennessee Quality Care, we are seeking a compassionate PT to join our Home Health team on a PRN basis. Why You’ll Love Working With Us Competitive pay Mileage reimbursement Flexible, PRN schedule (Monday–Friday, as needed) 401(k) with company match Supportive, team-oriented culture What We Offer Positive and collaborative team environment Comprehensive benefits effective the first of the month 401(k) retirement plan with generous company match Generous paid time off accruals Paid holidays Mileage reimbursement Tuition reimbursement Employee referral program Merit-based increases Employee discount programs Work-life balance What You’ll Do Provide physical therapy services according to a physician’s plan of care and state practice guidelines Evaluate patient function and assist in developing individualized care plans Educate patients, families, and caregivers on treatment plans and therapeutic activities Translate prescribed exercises into functional activities and activities of daily living Recommend modifications to equipment and home environments to support patient safety and independence Collaborate with physicians and interdisciplinary team members to coordinate care and monitor patient progress Document clinical notes, progress reports, and summaries in a timely and accurate manner Support and motivate patients and families throughout the rehabilitation process Supervise Physical Therapist Assistants in accordance with company policy and state regulations Maintain accurate clinical records and track patient goals and outcomes Qualifications Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA) Current Physical Therapist (PT) license in the state of practice At least two (2) years of experience in an acute care or similar setting (preferred) Current CPR and First Aid certification Reliable transportation, valid driver’s license, and current automobile insurance #J-18808-Ljbffr Addus HomeCare
