Care Transition Coordinator

To support patient transitions effectively, the full-time Care Transition Coordinator will work remotely to manage referral coordination, review medical records for eligibility, and ensure patients receive appropriate care in a timely manner. Key responsibilities Review medical records against clinical criteria to determine care eligibility Document and track patient referrals in company systems, collaborating with Care Transitions Nurses Educate patients and families about Home Health, Hospice, and Palliative Care services Required qualifications Call center experience in a healthcare setting Minimum of 1 year of healthcare or healthcare call center experience Associate degree preferred Strong computer skills and familiarity with multiple software applications Knowledge of Home Health, Hospice, and Palliative Care services
