Responsibilities
- Conduct initial and ongoing PT assessments for residents in a geriatric long-term care setting.
- Develop and implement individualized treatment plans focusing on mobility, strength, gait, balance, and functional independence.
- Provide direct patient care including therapeutic exercises, gait training, range of motion work, and pain management modalities as appropriate.
- Document progress notes, update plans of care, and communicate with the interdisciplinary team to adjust treatment as needed.
- Educate residents and families on home programs, fall prevention, and safety strategies.
- Monitor outcomes and ensure adherence to facility policies, regulatory standards, and documentation requirements.

Requirements
- Active physical therapy license in Pennsylvania (or eligibility for licensure in PA)
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Ability to work in a long-term care / skilled nursing facility setting
- Eligibility for a travel assignment and ability to commit to a 12-week contract with guaranteed hours
- Strong communication and teamwork skills; proficient in documentation and interprofessional collaboration
