We're looking for a highly organized, detail-oriented Case Transition Coordinator to join our Care Transitions team and play a key role in ensuring our residents receive the services they need while supporting positive clinical and financial outcomes. This position will include evenings and weekends.

As a Case Transition Coordinator, you'll serve as a vital link between our interdisciplinary teams, insurance providers, and care transition staff to ensure timely authorizations, effective communication, and seamless coordination of resident care.

Responsibilities include:

• Coordinate Managed Care and MLTC authorization processes from admission through discharge.

• Review admissions and insurance notifications to obtain and track authorizations.

• Maintain and update authorization tracking systems and spreadsheets.

• Gather and submit clinical documentation from EMR systems to insurance companies within required timelines.

• Coordinate communication between Admissions, Social Services, MDS, Therapy, Business Office, and clinical teams.

• Manage insurance appeals, reconsiderations, and level upgrade requests.

• Track authorization outcomes, denials, approvals, and key performance metrics.

• Monitor discharge planning and required documentation.

• Communicate review outcomes and authorization updates to facility interdisciplinary teams.

• Build positive working relationships with insurance representatives and facility staff.

• Perform additional duties as assigned.

• Knowledge of medical terminology.

• Experience working with Electronic Medical Records (EMR) systems.

• Proficiency with Microsoft Office, especially Excel.

• Experience with electronic faxing and document management.

• Strong communication and organizational skills.

• Exceptional attention to detail with the ability to manage multiple cases simultaneously.

• Ability to meet strict deadlines and prioritize daily tasks.

• Experience in skilled nursing, rehabilitation, managed care, utilization review, case management, or healthcare coordination preferred.
