[Required] - Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures, and other instructions furnished in written, oral, diagram, or schedule form. [Required] - Ability to follow standard precaution guidelines for infection control, including when providing patient care. [Required] - Demonstrates leadership skills and the ability to innovate and identify discharge options for medically complex patients. [Required] - Knowledge of the insurance industry and government reimbursement for healthcare settings. [Required] - Knowledge of community resources and post-acute care programs across the continuum. [Preferred] - Understanding of CMS Conditions of Participation for Discharge Planning. [Preferred] **Education:** - Associate's of Nursing [Required] - Bachelor's of Nursing [Preferred] **Work Experience:** - 2+ of hospital nursing experience [Preferred] - Specific experience in Case Management and discharge planning.
