The Center for Living & Rehabilitation is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team! Responsibilities:
• Provide high-quality, patient-centered care to our residents, including assessing, planning, implementing, and evaluating individualized care plans
• Collaborate with the interdisciplinary care team to develop and implement treatment plans that meet the unique needs of each resident
• Administer medications, injections, and vaccinations as ordered by physicians
• Monitor and record vital signs, health status, and medication administration
• Perform basic vital signs, such as taking temperatures, blood pressures, and pulse rates
• Assist with daily living activities, such as bathing, dressing, and grooming
• Participate in quality improvement initiatives to continually improve care delivery
