Compassionate Hospice RN Case Manager — On the Road
JOL Healthcare, Inc.
The Hospice RN Case Manager at JOL Healthcare, Inc. coordinates and delivers high-quality hospice care to patients in homes and facility settings. You will develop individualized plans, manage visits, and collaborate with an interdisciplinary team while ensuring compliance with hospice standards. You will document all care in the EMR, educate families on disease progression, and serve as the primary contact for patients and families.
