Community-Based Visiting Nurse (RN)

Community-Based Visiting Nurse (RN) Do you hold a current RN license and enjoy independent, community-based nursing? Are you skilled at assessment and meticulous documentation? Do you have reliable transportation and a valid driver’s license for local home visits? Have you worked in home health before (or are eager to transition)? What will you do each shift? Assess: Conduct thorough in-home patient assessments Plan: Develop and update individualized care plans Monitor: Observe condition changes and document clearly and promptly Coordinate: Communicate with physicians, caregivers, and the interdisciplinary team Teach: Provide practical education to patients and families Role essentials Schedule: Part‑time, 16–24 hours per week Patient load: Approximately 2–3 patients per day Pay: $40–$43 per hour Preferred background Home health experience is a plus Ready to apply?
