About the Role:

As a Licensed Practical Nurse (LPN), you will be responsible for providing direct patient care to our residents, including administering medications, performing vital signs, and assisting with daily living activities. You will also communicate effectively with our interdisciplinary team to ensure that our residents receive comprehensive and individualized care.

Responsibilities:

• Deliver high-quality patient care to residents, including administering medications, performing vital signs, and assisting with daily living activities

• Conduct routine assessments and monitor resident progress to identify any changes or concerns

• Develop and implement individualized care plans in collaboration with our interdisciplinary team

• Provide emotional support and maintain a positive, respectful attitude towards our residents

• Maintain accurate and up-to-date records of patient care and treatment plans

• Participate in quality improvement initiatives to continuously improve our care delivery processes

Requirements:

• Must hold a current LPN license in the State of Massachusetts

• Experience in long-term care or skilled nursing setting

• Strong communication, interpersonal, and problem-solving skills

• Ability to work effectively in a fast-paced environment and prioritize tasks efficiently
