- Conduct individualized activity assessments to identify interests, abilities, and preferences while supporting personalized treatment goals and care plans in a skilled nursing setting.
- Facilitate and actively participate in recreational, therapeutic, social, and group activities that promote resident wellness, independence, and quality of life.
- Ensure activity areas, equipment, and supplies are clean, safe, well-stocked, and organized to provide a welcoming environment for residents and staff.
- Complete clerical duties including documentation, attendance records, activity calendars, supply inventory, and other administrative tasks as assigned to support the Activities Department.
- One or more of the following required:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
- Heart Saver AED obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
- High school diploma or GED preferred.
