RN Case Manager Full Time

We are excited to welcome a dedicated and skilled Registered Nurse to our healthcare team as a Case Manager. This full-time position offers the opportunity to significantly impact patient care and well-being in a collaborative and supportive environment.

Key Responsibilities:

• Deliver exceptional patient care to diverse populations, considering various health conditions and age groups.
• Perform comprehensive assessments and accurately document patient histories.
• Create and implement personalized care plans tailored to individual patient needs.
• Administer prescribed medications and treatments, monitoring patient progress and recording vital signs.
• Work closely with a multidisciplinary team to ensure integrated care and achieve optimal patient outcomes.
• Provide education to patients and families on health management, disease prevention, and available treatment options.
• Maintain meticulous patient records and documentation.

Qualifications:

• With Home health experience as RN
• Knows Oasis but not required
• Graduate of an accredited nursing program; BSN, PHN preferred.
• Current Registered Nurse license for practice in the relevant state(s).
• A minimum of one year of experience in an acute care setting or equivalent experience.
• Valid driver's license with insured and well-maintained vehicle as per state and agency guidelines.
• Current CPR certification.
• Excellent communication and interpersonal skills.
