Description: The RN implements professional nursing practice centered on the patient as a whole person. A caring environment focused on healing and wellness is created. Nursing practices are based on evidence and current research as well as policies and procedures. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. The nurse coordinates and provides direct patient care activities on the unit. This includes provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is part of the role. The RN provides patient and family-centered education. Care coordination and individualized treatment planning is performed as applicable. A comprehensive assessment is performed and documented in accordance with scope of practice and policies. The patient's culture, age, and developmental level are considered. Patient risk factors and presenting complaints are identified. Health information is collected from family or others as necessary. Both objective and subjective data are gathered. The initial admission assessment is verified if obtained by others in the healthcare team. A problem list is developed, prioritizing patient needs based on inter-professional team input. Actions and interventions relevant to the patient’s culture and age are designed. Desired outcomes are determined in coordination with the patient and family. Patient care therapies and procedures are fully implemented. Procedures, treatments, and processes are explained to the patient and family. Initiatives are taken to alleviate the patient’s stress and discomfort. Changes in patient condition are recognized, responded to, and communicated appropriately. Ongoing assessment and response to procedures and therapies are conducted and documented. Tasks are delegated appropriately, with oversight provided to team members. Accuracy and completeness of delegated patient care are monitored. Educational needs of patients are identified and addressed related to prioritized problems. The patient discharge process is facilitated by executing discharge orders in a timely manner. Potential discharges are identified, and discharge teaching is performed. Coordination of the discharge plan with Case Management and others is ensured. Input into patient/nurse assignments is provided, considering staffing and patient acuity. Active participation in unit-based activities supporting organizational values and priorities is demonstrated. A positive group dynamic and healing environment are promoted through words and actions. Nursing excellence is assured in accordance with accepted standards and evidence-based practice. Active participation in department and unit quality improvement activities is expected.
