Responsibilities

• Conduct comprehensive occupational therapy assessments in home health settings.

• Develop and implement individualized home health OT treatment plans.

• Document treatment plans, progress notes, and outcomes in electronic health records.

• Maintain compliant home health practices, including infection control and safety standards.

• Set and track patient progress against therapy goals and milestones.

• Educate patients and caregivers on therapy techniques and safe practices at home.

• Demonstrate value through data-driven reporting on caseload and outcomes.

• Coordinate care with multidisciplinary team and referring physicians.

• Identify barriers to access and facilitate resources to support home-based therapy.

• Drive patient centric care by prioritizing outcomes that align with organizational goals.

• Proactively seek professional development opportunities to advance OT expertise.

Experience/Qualifications

• Licensed Occupational Therapist with current state OT registration

• Home health OT experience and familiarity with home safety protocols

• Ability to perform comprehensive home assessments and develop PRN treatment plans

• Proficient documentation in electronic health records and progress tracking

• Infection control, safety standards, and compliant home health practices

• Data-driven reporting skills to demonstrate caseload outcomes

• Strong communication with patients, caregivers, and multidisciplinary teams

• Continuous professional development; willingness to pursue advanced OT expertise

• Ability to identify barriers to access and coordinate community resources

• Goal-oriented mindset with ability to set and monitor therapy milestones
