Part Time Ambulatory RN Care Coordinator

Are you a compassionate nurse who loves working in pediatrics? Lee Physician Group has an opening for a Part Time Ambulatory RN Care Coordinator. This is an in office position and the Care Coordinator is responsible for assessments and care planning/coordinating. Case management and patient education, cleft palate team and care transition. This position does require 3 years of RN experience.

The Ambulatory RN Care Coordinator utilizes a collaborative process of assessment, planning, implementation and evaluation to engage, educate, promote and maintain optimal health status of assigned patients. This position functions in a collaborative partnership with the patient, physician and the care team in the office setting. This position serves in an advocacy role to ensure the delivery of quality, efficient, and cost-effective patient-centric health care services. The Ambulatory RN Care Coordinators case load consists of moderate to high risk patients.

Educational Requirements

Degree/Diploma Obtained Program of Study Required/ Preferred and/or Associate's Nursing Required and Bachelor's Nursing Preferred

Experience Requirements

Minimum Years Required Area of Experience Required/ Preferred and/or 3 Years Clinical Nursing Required

Additional Requirements

Minimum of 3 years in clinical setting with emphasis on patient education and chronic disease management; home health, skilled nursing, medical surgical nursing required. Care coordination experience preferred.
