Sr Case Manager/Case Manager, Ryer Avenue About this position Position: Senior Case Manager/Case Manager, Ryer Avenue Function: Provides case coordination and delivers community rehabilitation services. Reports to: Program Director Location: The Bronx (Mount Hope) Schedule(s): Mondays‑Fridays (11:00am-7:00pm) or Sundays‑Thursdays (12:00pm-8:00pm) Tasks: Conduct home visits on a regular basis for assigned case load. Obtain psychosocial histories and conduct functional skills assessments. Establish and maintain liaison with primary provider of psychiatric care. Ensure access to psychiatric, substance use, medical services, and entitlement benefits. Monitor client medication, including adherence and side‑effects. Administer standardized health screening tools. Develop individualized rehabilitation plan in accordance with client needs, strengths, and goals. Deliver community rehabilitation services such as Assertiveness/Self Advocacy Training, Community Integration Services, Daily Living Skills Training, Health Services, Medication Management Training, Parenting Training, Rehabilitation Counseling, Skill Development Services, Socialization, Substance Abuse Services, and Symptom Management. Provide housing support services such as education/employment support, behavioral health, harm reduction, assistance with entitlements, health care, emergency services, Health Home Care Coordination, and transportation. Document client response to service, progress and regressions, and regularly update the plan. Train clients in safety procedures, conduct fire drills and self‑preservation tests. Prepare residents for transition to independent living and make necessary referrals. Facilitate crisis intervention in emergencies. Staff emergency phone on a rotating basis. Maintain client charts and submit necessary notes and reports, including incident reports, in compliance with agency procedures. Oversee apartment maintenance, inventory furniture, and alert program director of maintenance problems. Attend in‑service training as requested by supervisory personnel. Provide coverage as needed for other staff on the program team. Serve as “Qualified Mental Health Staff Person”. Perform duties as assigned by supervisor. Requirements: Senior Case Manager Qualifications: M.A. in psychology or a human services‑related field. B.A. in psychology or a human services‑related field with a minimum of 3 years of experience providing direct care in human services. Experience working with adults with serious mental illness preferred. Strong written and verbal communication skills. Master’s Degree preferred. Strong written and verbal communication skills. Compensation: Single Site SCM Salary: $60,355 plus generous benefits. Single Site CM Salary: $52,081 plus generous benefits.
