Registered Nurse – Home Health

Schedule: 8 hours per day

Assignment Details:

• Contract Length: 9 weeks
• Guaranteed Hours: 40 per week

Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• 2+ years of home health experience
• 1-2 years of experience as a Registered Nurse in a clinical care setting
• Proof of current CPR certification
• Current driver's license
• Reliable transportation and insurance

Roles & Responsibilities:

• Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs.
• Implement and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and their families regarding care, medications, and home safety.
• Supervise and provide clinical direction to support staff to ensure quality of services provided.
• Monitor assigned cases for compliance with third-party payer requirements.
• Perform on-call responsibilities to support patients and families as necessary.

Compliance / Notes:

• Maintain current licensure certifications and continue education requirements.
• Ability to work flexible schedule and travel locally.
• Maintain confidentiality in all aspects of care.
