Job Description:

As an LVN/LPN in our Behavioral Health unit, you will play a critical role in the care and support of our patients. Your responsibilities will include, but are not limited to, the following:

• Utilizing the nursing process to assess, implement, and evaluate patient/family care.
• Providing nursing care, including the administration of medication, to meet patients' needs.
• Participating in the development and maintenance of a safe and therapeutic environment in the assigned care area.
• Adhering to and implementing organizational and departmental policies and procedures as they pertain to the work area.
• Performing constant observations, intermittent observations, specializing checks, or other forms of patient safety procedures as assigned.
• Sharing information appropriately with members of the multidisciplinary team and other hospital department representatives through verbal reports and written documentation.
• Participating in pre-admission evaluations as requested by the Program Manager and completing Admission Nursing Assessments for all new Admissions to C/A Residential I & Partial Programs (Including ARCC).
• Conducting monthly medication renewals and reviewing daily reports.
• Being responsible for follow-up on medical tests and labs, overseeing the completion of admission paperwork, and completing the medication portion of aftercare forms.
• Tracking medical errors to ensure patient safety.

Qualifications:

• Psych/Behavioral Health Experience - Required
• Experience working with child and adolescent populations - Required
• CPI Certification - Required (Candidates willing to obtain onsite certification are acceptable)
• COVID vaccination and Booster - Required

Certifications and Licenses:

• BLS Certification
• CPI Certification
• Vermont State Nursing License (must be active at the time of submission)
