Description

• Assess patients within both inpatient and outpatient environments and formulate customized treatment plans
• Deliver physical therapy interventions targeting mobility, strength, balance, and overall functional enhancement
• Team up with nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes
• Instruct patients and their families on therapeutic objectives, post-discharge planning, and home exercise routines
• Maintain thorough documentation of evaluations, treatments, and progress in line with facility and regulatory standards
• Engage in interdisciplinary care team meetings to foster coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is highly desired
• Additional specialized certifications (e.g., in neurology, geriatrics) are a bonus but not mandatory
