Family Medicine Physician - Outpatient Opportunities | Indiana
\nA respected community-based healthcare organization in Indiana is actively seeking Board-Certified/Board-Eligible Family Medicine Physicians for multiple full-time outpatient opportunities across several clinic locations.
\nThese openings offer a mix of traditional outpatient Family Medicine roles and optional academic/faculty opportunities for physicians interested in teaching residents.

\n• Mishawaka, IN - Outpatient Family Medicine
\n• Michigan City, IN - Outpatient + Residency Faculty Opportunity
\n• Valparaiso, IN - Outpatient + Residency Faculty Opportunity

\nFaculty Opportunities (Michigan City & Valparaiso Only)
\nPhysicians interested in academic medicine will have the opportunity to:
\n• Teach and mentor Family Medicine residents
\n• Participate in residency program activities
\n• Assist with resident evaluations and recruitment
\n• Combine clinical practice with academic involvement
\nCore Responsibilities
\n• Provide comprehensive Family Medicine care for patients across the lifespan
\n• Diagnose and manage acute and chronic conditions
\n• Perform wellness visits and preventive screenings
\n• Coordinate care with specialists and interdisciplinary teams
\n• Maintain accurate and timely documentation
\n• Participate in quality improvement initiatives
\n• Collaborate with behavioral health and support services teams
\nQualifications
\n• MD or DO from an accredited medical school
\n• Completion of Family Medicine residency program
\n• Board Certified or Board Eligible in Family Medicine
\n• Active or eligible Indiana medical license
\n• DEA certification
\n• CPR certification
\n• Strong communication and patient-centered care skills
\nPreferred
\n• Prior outpatient or community health experience
\n• Interest in teaching/residency training for faculty-track positions
\n• Ability to thrive in a collaborative care environment
