Job Summary:

The Patient Care Coordinator serves as the primary point of contact for patients, providers, and referring physicians to ensure seamless coordination of patient care. This role focuses on scheduling, referrals, insurance coordination, medical documentation, and patient advocacy while delivering exceptional customer service in a fast-paced primary care clinic.

Primary Responsibilities:

• Coordinate patient care from referral through treatment.
• Schedule appointments, surgeries, procedures, and diagnostic testing.
• Coordinate pre-admission testing and pre-operative appointments.
• Process referrals and ensure timely follow-up.
• Verify insurance authorizations and financial information.
• Maintain complete and accurate medical records.
• Communicate with physicians, nurses, patients, families, and outside providers.
• Track referral status and provide reports to leadership.
• Support quality improvement initiatives.
• Maintain patient confidentiality (HIPAA).
• Deliver excellent customer service using AIDET principles.

Required Qualifications:

• Minimum 1 year of experience in a healthcare/medical setting
  OR
• 2 years of customer service experience
• Experience handling confidential information.
• Experience working in a fast-paced environment.

Preferred Qualifications:

• Electronic Medical Records (EMR) experience.
• Medical office or clinic experience.
