Nurse Practitioner, Advanced Practice Provider

Role Overview The purpose of a Nurse Practitioner at Oak Street Health is to provide effective and equitable value-based primary care to adults on Medicare, keeping patients happy, healthy, and out of the hospital. Nurse Practitioners operate independently and collaborate with physicians and the larger care team, delivering longitudinal care and building trust. Core Responsibilities Provide exceptional primary care. Conduct office visits for routine and acute issues. Administer annual wellness visits and health risk assessments. Coordinate care with other providers, specialists, testing facilities, and agencies. Lead population health initiatives, such as ensuring eligible females receive breast cancer screening every two years. Assist with phone triage, outreach, and home visits as needed. Educate patients about their conditions, care plans, and treatments. Participate in Oak Street Health promotional activities. Other duties as assigned. Required Qualifications Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate. National certification in at least one specialty: Family Nurse Practitioner, Adult‑Gerontology Primary Care, Nurse Practitioner Adult, Gerontological Nurse Practitioner. Active, non‑probationary state Nurse Practitioner license and active DEA license. US work authorization. Preferred Qualifications Experience in primary care (internal medicine, geriatrics, or family medicine). Knowledge of HCC documentation, ICD‑10 coding, and Health Risk Assessments. Passion for teamwork and cross‑functional collaboration. Desire to engage in an innovative, empirically‑guided population health model. Bilingual proficiency in applicable language(s).
