Job Description:

The RN, Case Manager will work with a team in the acute inpatient setting, responsible for case management and discharge planning across various hospital units. Candidates must have strong computer skills and experience with Epic EMR. Minimal training will be provided, requiring the ability to rapidly assimilate the role and function efficiently.

Responsibilities:

• Manage care across all inpatient units throughout the hospital
• Collaborate with a team of social workers for patient care
• Handle a case load of 15-20 patients per day
• Consult with respiratory therapy and pharmacy for discharge planning as needed

Requirements:

• Minimum 3 years of acute inpatient case management experience in a large hospital (500+ beds, Level 1/Academic Medical Center)
• Experience with travel in a Level 1 facility
• Proficient in Epic EMR and strong computer skills
• Nursing degree preferred: RN, BSN (ADN considered), BLS certification through AHA, CCM preferred
• Recent (within the last two years) acute inpatient case management and discharge planning experience
