Occupational Therapist / OT

Job Title: Occupational Therapist (OT) – Full-Time Location : Walnut Creek, CA At Rehab Without Walls®, we bring therapy directly to patients' homes and community settings, providing functional, real‑world rehabilitation. We are seeking an experienced Occupational Therapist to join our full‑time Home & Community Neurorehabilitation team. Key Responsibilities Serve as the primary therapist from evaluation through discharge, rebuilding patients' independence through real‑life activities. Perform evaluations and develop treatment plans. Regularly reassess effectiveness of treatment plans, attend staffing meetings and family conferences. Conduct discharge evaluations, make safety recommendations, and provide referral/education resources. Communicate patient needs and progress to the treatment team, physicians, service recipients, and family members. Coordinate with other disciplines to ensure collaboration and enhance patient outcomes. Qualifications Minimum of a bachelor’s degree in Occupational Therapy from an accredited program. Current, unrestricted license as an Occupational Therapist in the state of practice. Current CPR certification. At least one year of experience as an Occupational Therapist. Knowledge of rehabilitation techniques related to complex neurological diagnoses preferred. Tasks may include pushing/pulling, bending/stooping, reaching, kneeling, frequent positioning. Physical requirements may be confirmed via health screen/examination; accommodations may be provided.
