Job DescriptionJob DescriptionDescriptionAs a Case Manager, you'll build meaningful relationships with children, families, schools, providers, and community partners while helping connect people to the services and resources they need.This is more than paperwork and referrals. You'll help families understand their options, coordinate support, identify barriers, and advocate for the people we serve.What You'll DoEvery family has different needs. You'll work collaboratively to understand those needs, coordinate services, and help create a path forward.Provide ongoing case management and service coordinationBuild positive, trusting relationships with individuals and familiesGather information from families, schools, medical providers, and community agenciesHelp families understand available programs, resources, and servicesScreen referrals and connect families to appropriate servicesCoordinate communication between families, providers, agencies, and other professionalsIdentify gaps or barriers in services and help develop solutionsAdvocate for the needs of individuals and familiesProvide education, support, and technical assistance to families, providers, and community agenciesDevelop and maintain strong relationships with community partnersAttend team, case, agency, and other required meetingsMaintain accurate, confidential case records and documentationPrepare reports and other documentation related to case management and service deliveryMaintain documentation required for court proceedings when applicableComplete required documentation accurately and within three business daysWhat Makes This Work Different?Families may come to us feeling overwhelmed by services, paperwork, appointments, and systems that can be difficult to navigate.
