Hospice RN Case Manager: Full Time

Make a difference every day as a Hospice RN Case Manager—deliver compassionate care, guide families through end‑of‑life journeys, and collaborate with an interdisciplinary team. Summary of Responsibilities: The Hospice RN Case Manager reports to the Hospice Clinical Director and provides nursing care and case management to patients and families within and outside of The Highlands at Wyomissing campus. Implements nursing interventions and coordinates patient/family interdisciplinary plans of care, and supervises personnel to deliver care and coordinate care delivery. Essential Functions/Duties: Completes an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illnesses. Provides professional nursing care by utilizing all elements of the nursing process. Assesses and evaluates patient’s status by: Writing and initiating plan of care. Regularly re‑evaluating patient and family/caregiver needs. Participating in revising the plan of care as necessary. Using health assessment data to determine nursing diagnosis. In conjunction with the Interdisciplinary Group (IDG) develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process. In conjunction with the Physician, adds medications and treatments as prescribed by the physician to the patient’s plan of care. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient/family as appropriate per assessment and plan. Participates in on‑call duties as defined by the on‑call policy. Ensures that arrangements for equipment and other necessary items and services are available. Supervises ancillary personnel and delegates responsibilities when required. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in‑service classes. Fulfills the obligation of requested and/or accepted case assignments. Actively participates in quality assessment performance improvement teams and activities. Participates in and completes all required ongoing training, including monthly online and in‑person training, annual skills competencies education, and role‑specific certifications, to ensure continued competence and adherence to organizational and regulatory standards. Required Skills/Abilities: Familiarity with Microsoft Outlook, Surface Pro Tablet, iPhone. Ability to learn and navigate EMR system. Clear, concise, and effective communication within the hospice team, patient/family dynamic, and pertinent healthcare professionals. Ability to provide education to patients and family members regarding death and the dying process in a compassionate manner. Ability to adapt to changes in patient acuity, changing priorities and managing unexpected situations. Knowledge of end‑of‑life symptom management and pharmacological and non‑pharmacological interventions. Self‑directed and able to work with minimal supervision. Demonstrates excellent observation, problem solving, verbal and written communications, nursing skills per competency checklist. Shows ability to organize and prioritize workload independently. Position Type/Expected Hours of Work: Position is Monday through Friday with on‑call expectations after hours, weekends and holidays. Standard hours may fluctuate based on caseload. Standard days and hours of work for standard Hospice business hours are from 8:00 am to 4:30 pm. Hours worked after hours weekdays are from 4:30 pm to 8:00 am Monday through Thursday. Weekends/holidays are Friday to Monday from 8:00 am to 8:00 am. Holidays are covered on a rotating schedule. Education and Experience Requirements: High school Diploma or GED (Required) PA Registered Nurse License (Required)Bachelor’s degree in nursing from an accredited institution (Preferred) Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order. A minimum of two years’ experience, at least one of which is in public health, home care, or hospice nursing (Preferred) CPR Certification (Required) Completion of the Highlands Hearthstone Ambassador training within 30 days of employment (Required) Work Authorization/Security Clearances: Medical Screening Criminal History Clearance FBI Fingerprint Clearance (Only required if you have not lived in PA for the last 2 years) Immigration and naturalization (I-9) Clearance Valid Pennsylvania Driver's License and acceptable MVR (if operating company vehicle) Physical Requirements and Physical Demands: While performing duties of this job, the employee is occasionally required to climb, kneel, pull with force, reach above head, reach above shoulder, push/pull (up to 100 lbs.), lift from floor level up, lift from waist level up, lift above shoulders/head, ability to lift, push, pull with assistance (mechanical or co‑worker) the weight of the average patient specific to department, Lifting 25 lbs. maximum and frequent lifting and/or carrying of objects weighing up to 10 lbs.
