You'll work closely with families and caregivers to ensure that each person receives the necessary resources and support to thrive. We are looking for four Supports Coordinators to cover one of each of the following areas: Clairton, North Hills, South Hills, and Washington County About Familylinks: For over six decades, Familylinks has served the diverse communities of Western Pennsylvania. Familylinks offers family-centered services in Behavioral Health, Service Coordination, Youth and Family, and Aging Services. Supports Coordinator Responsibilities : All essential duties and responsibilities are completed within regulatory timeframes. Complete timely and thorough documentation after each contact/service provided. Meet productively goals and report accurately per departmental policy. As requested, assist in emergency situations. Attend supervision/meetings/training as requested/required. Accurately record time and attendance via HRIS and Timekeeping system. Document and report monthly expenses accurately and within agency policy expectations. Model the organization's mission and values. Other duties as assigned. Supports Coordinator Requirements: Bachelor's degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science*; OR 2 or more years' experience as a County Social Service Aide 3 and two years of college level course work, which include at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service*; OR Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service* and 1 or more years of experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions**; OR A combination of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science and two years' professional experience in developmental disabilities, special education, mental health, counseling psychology, school psychology, social work or health and rehabilitative services. A valid driver's license along with access to a vehicle. Proficiency with MS Office, computer/laptop, tablet and cell phone required. Working Conditions: Flexible scheduling may be required. Must be able to travel to the various office and community locations- field work makes up 60% of this role. Work will be completed remotely at home, within the office, or other community location maintaining HIPAA compliance- 40% of this role is office based. Ability to lift and carry light boxes of files and supplies (less than 10 pounds) on occasion. A limited amount of walking and standing may be necessary in carrying out duties.
