- Evaluate and treat patients in a skilled nursing facility setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward therapy goals
- Collaborate with interdisciplinary care team to ensure optimal patient outcomes
- Educate patients and caregivers on therapeutic exercises and home programs
- Active Physical Therapist (PT) license in the state of South Carolina
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Prior experience working in a skilled nursing facility or long-term care setting
- Familiarity with MDS documentation and Medicare guidelines
- Strong communication and team collaboration skills
- Opportunity to make a meaningful impact on patient recovery and quality of life in a supportive SNF environment
- Gain valuable contract experience with a defined 13-week engagement starting in November
