Case Manager

Overview Position Title: Case Manager — Reports to: Case Manager Supervisor / DSS Department: Social Services Employment Status: Full-Time Responsibilities Carry out case management activities as per the Part 900 Regulations and the Department of Homeless Services (DHS) Case Management Guidelines for family shelters. Conduct assessments and obtain required documents (e.g., birth certificates, social security cards, picture IDs). Show the Safe Sleep Video and complete the appropriate documentation. Ensure clients sign the Code of Conduct and the facility rules and regulations. Obtain proof of employment and assist with obtaining entitlements. Maintain clients’ records by reviewing case notes, logging events and program progress. Communicate clients’ progress by conducting weekly interdisciplinary meetings and evaluations. Handle intakes of adults and ensure that all documentation is accurate and properly filed. Recommend action plans and develop an Independent Living Programs (ILP) for clients, reviewed and updated on a weekly basis. Collaborate with the Housing Specialist to create an exit strategy and target exit date. Assist families with seeking and accepting housing. Meet target goals set for each client by the Housing Specialist. Conduct weekly unit inspections. Maintain detailed records of each client in the form of progress notes. Provide the Director of Social Services (DSS) / Case Manager Supervisor with reports and summaries of caseload for management review. Liaise with management, communicating clients’ progress and any case concerns. In the event of an emergency, Essential Employees are required to report to work as scheduled. Duties and responsibilities are subject to change based upon assessment of company needs. Qualifications Bachelor's Degree in Psychology, Social Work, Sociology, Mental Health, Nursing, Human Services, or Education Services required. Minimum of 2 years’ experience working within the Social Services field. Strong interpersonal skills and ability to communicate effectively (verbal and written). Must be organized. Experience with DHS Cares / Homeless population is a plus. #J-18808-Ljbffr
