Medication Administration – Administer medications and treatments as prescribed; monitor for reactions. Wound Care – Provide wound care management, dressing changes, infection control, and monitor healing. Patient Education – Educate patients and families about conditions, medications, and self‑care techniques. Monitoring and Evaluation – Monitor progress, evaluate effectiveness of the care plan, and adjust as needed. Collaboration – Coordinate care with physicians, therapists, social workers, and home‑health aides. Documentation – Maintain accurate medical records and documentation of assessments, interventions, and responses. Support and Advocacy – Provide emotional support and advocacy; respect patients’ rights and preferences. Qualifications Graduate of an approved school of nursing or Bachelor’s degree in Nursing from an accredited college/university. Currently licensed, in good standing, to practice in New York as a Registered Nurse. Minimum 1 year community health nursing experience or equivalent professional experience. Knowledge of nursing and medical practices, procedures, regulations and guidelines pertaining to home care, including OASIS. Compliance with accepted professional standards and practice.
