Additionally, our Rehab Center is nationally ranked by UDSMR and accredited by CARF for outstanding rehabilitation programsCare Management within the Department of Psychiatry includes a collaborative practice with patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. Social workers facilitate communication and care coordination along a continuum through effective transitional care management. Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.). Your voice matters here, because we know that to inspire and retain the best people, we must empower them.Required qualifications:Master's Degree in Social Work.Coursework/Training in All LCSWs must follow the guideline by Los Angeles County Department of Mental Health for LPS designation and keep their designation in current status.Upon hire: California Clinical Social Worker License Within 18 months of hire: Designated as Behavioral Health Clinician to perform 5150 evaluationsWithin 1 year of hire: CPI certification (nonviolent crisis intervention training)2 years of Healthcare related setting with psychiatric mental health patient population in area of service, preferably in an inpatient setting.Computer experience.Preferred qualifications:Discharge planning experience.Why Join Providence?
