Care Manager Opportunity at Partnership for Children of Essex (PCE)Partnership for Children of Essex (PCE) is currently seeking care managers! We are looking for exceptional individuals who are motivated, compassionate, and committed to providing outstanding service each and every day to children and families throughout Essex County.At PCE, our care managers play a critical role in supporting youth and families through a collaborative, strength-based wraparound approach focused on stability, empowerment, and long-term success.Key responsibilities include:Utilize a team-based approach in facilitating child and family team meetingsProvide face-to-face contact with youth and families in the home and community, in addition to ongoing telephone communicationParticipate in crisis on-call coverage as needed (PCE provides 24/7 on-call crisis services for all PCE clients)Coordinate and facilitate comprehensive assessments of the strengths and needs of children and familiesCollaborate in the implementation and development of individual service plans (ISPs) utilizing the wraparound model of careCoordinate care management services across multiple systems and providersDevelop and maintain strong community relationships and resource connections for familiesQualifications and skills:Bachelor's degree in a related field such as social work, counseling, psychology, criminal justice, or special education required; master's degree preferredMinimum of one year of related experience in human services, case management, social work, or a similar roleExperience with community relations and resource development preferredPrior experience managing wraparound services is a plusBilingual candidates (Spanish, Portuguese, and/or Creole) are strongly encouraged to applyExcellent written and verbal communication skills requiredMust be computer literateKnowledge of the Essex County community preferredValid driver's license requiredWhy Join PCE?The Partnership for Children of Essex (PCE) is a non-profit organization dedicated to creating a pathway for hope and improving the quality of life for youth and families in our community.We offer an excellent benefits package including:Medical, dental, and vision coverage403(b) retirement planGenerous PTOLife insuranceFlexible scheduleHybrid work opportunitiesIf you are passionate about making a meaningful impact in the lives of children and families, we would love to hear from you!PCE is an equal opportunity employer- M/F/D/V
