Social Work Manager

About the Role Our groundbreaking hospice and palliative care programs offer a broad range of services in the community or facility to help patients and their families ease the burden of managing a life‑limiting illness or end‑of‑life care. Responsibilities Mentor and provide leadership direction to the Social Work staff. Ensure Social Worker documentation compliance. Participate in referral and community education and presentations. Oversee the preparation and planning of weekly team meetings. Coordinate the provision of psychosocial services and maintain adequate staffing and appropriate patient assignments. Maintain integration of psychosocial services within the interdisciplinary team approach and act as a member of the management team. Qualifications Master's Degree in Social Work from a Graduate School of Social Work accredited by the Council of Social Work Education. Minimum five years post‑Master’s; minimum 2 years of end‑of‑life care experience; minimum 2 years of supervisory/program development experience. New York State LCSW licensure within one year of employment (R# preferred). SIFI Certification within one year of employment. Valid NYS driver’s license preferred.
