Home Care RN Case Manager
Full Time | Days | Salaried
We have two great Full Time opportunities to join our team as an RN Case Manager:
1) Territory covers Warwick, Cranston, East Greenwich and surrounding areas
2) Territory covers Cranston, Johnston, North Providence, Lincoln and surrounding areas
*Travel outside of territory will be required based on agency needs.

The Home Care RN Case Manager contributes to our Mission of Caring by providing compassionate direct patient care to our clients in a one-on-one setting in their homes. The Home Care RN Case Manager conducts nursing visits to include admissions, revisits, and discharges while providing assessments and education to patients. The Home Care RN Case Manager for Saint Elizabeth Home Care is a part of a collaborative interdisciplinary team with ample administrative support. Our Home Care RN Case Manager is equipped with point of care devices (iPad), as well as all necessary and requested medical equipment and supplies.
Qualifications for a Home Care RN Case Manager:
- Current unencumbered RN license in Rhode Island.
- Minimum of one year nursing experience, home care experience preferred.
- CPR certification.
- Reliable transportation, current clean driver's license, automobile insurance.
