Case Manager RN

Registered Nurse (RN) Case Manager Location: Presque Isle, ME 04769 Job Type: Full-Time Schedule: 8:00 AM - 5:00 PM (Monday - Friday) Salary: Competitive and based on experience Position Summary We are seeking a compassionate and detail-oriented Registered Nurse (RN) Case Manager to join our healthcare team. The RN Case Manager will coordinate patient care across the continuum, ensuring quality, cost-effective, and patient-centered outcomes. This role involves collaborating with interdisciplinary teams, assessing patient needs, developing care plans, and facilitating access to appropriate healthcare resources and services. The ideal candidate is an experienced RN with strong clinical judgment, care coordination expertise, and a passion for improving patient outcomes. Key Responsibilities Conduct comprehensive patient assessments to identify medical, psychosocial, and discharge planning needs. Develop, implement, and monitor individualized care plans. Coordinate care among physicians, nurses, therapists, social workers, and other healthcare professionals. Facilitate transitions of care, including discharge planning and post-acute care placement. Educate patients and families regarding treatment plans, disease management, and available community resources. Monitor patient progress and adjust care plans as necessary. Ensure compliance with regulatory, accreditation, and payer requirements. Review medical records and documentation for medical necessity and quality standards. Advocate for patients to ensure appropriate services and resources are available. Participate in utilization management, care coordination, and quality improvement initiatives. Maintain accurate and timely documentation in the electronic medical record (EMR). Required Qualifications Active, unrestricted Registered Nurse (RN) license in the state of practice. Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN). Minimum of 2 years of clinical nursing experience. Strong knowledge of case management principles, discharge planning, and care coordination. Excellent communication, organizational, and critical thinking skills. Proficiency with electronic medical records and care management software. Preferred Qualifications BSN preferred. Previous Case Management experience in a hospital, health plan, home health, rehabilitation, or managed care setting. Certified Case Manager (CCM) certification or willingness to obtain. Experience with utilization review, managed care, Medicare, Medicaid, or value-based care models. Knowledge of community resources and post-acute care services. #J-18808-Ljbffr
