NOW HIRING – Case Management Coordinator Hialeah / Miami-Dade County, FL Competitive Pay | Mileage Reimbursement Provided Schedule: Monday–Friday | 8:00 AM – 5:00 PM 50–75% Local Travel Required Pay rate: $36.07/hr

In this role, you will conduct telephonic and face‑to‑face case management visits to assess, plan, coordinate, and support member healthcare needs while promoting overall wellness and quality outcomes. Key Responsibilities: Conduct member assessments and coordinate individualized care plans Manage supportive and medically complex caseloads Coordinate care with PCPs, specialists, and healthcare providers Assist with prior authorizations and medication reviews Connect members with community resources and support services Monitor short‑ and long‑term health outcomes Maintain accurate documentation and case management records Provide condition management education and wellness support Ideal Candidate: Strong case management and care coordination experience Excellent communication and organizational skills Comfortable working independently in the field Tech‑savvy with strong documentation skills Bilingual English/Spanish required Reliable transportation required for local travel Enjoy flexibility, autonomy, and the opportunity to make a direct impact on member health and wellness within your local community.
