Physical Therapist

As a Physical Therapist with our client, you will serve as a vital member of the Interdisciplinary Team (IDT). Your primary focus will be on care planning, coordination, and the delivery of restorative and rehabilitative physical therapy services to participants, helping them maximize their independence and safety. Key Responsibilities:

• Assessments: Conduct comprehensive Physical Therapy assessments in our facility and occasionally in participant homes to define care needs, preferences, and goals.
• Care Planning: Actively participate in IDT meetings to develop and refine individualized participant care plans.
• Interventions: Deliver and document treatments as outlined in care plans, including active/passive exercise, muscle re-education, gait and functional training for Activities of Daily Living (ADLs), and modalities involving massage, heat, sound, water, light, or electricity.
• Supervision: Oversee Therapy Assistants by providing the necessary training, instruction, and guidance to implement Functional Maintenance Programs (FMP).
• Education: Provide clear training and instruction to staff, participants, and caregivers or families on the proper use and maintenance of therapeutic equipment.

Qualifications & Requirements:

• Education: Bachelor's degree in Physical Therapy required; Master's or Doctoral degree preferred.
• Licensure: Valid California Physical Therapy License, or verifiable proof of license eligibility.
• Certifications: CPR and First Aid certification preferred upon hire (must be obtained within the first 180 days of employment and maintained thereafter).
