Description

• Conduct comprehensive evaluations in inpatient and outpatient settings while crafting personalized treatment plans
• Implement therapeutic interventions aimed at enhancing mobility, strength, balance, and overall function
• Collaborate with a multidisciplinary team including nurses, physicians, case managers, and rehab specialists to ensure optimal patient outcomes
• Educate patients and families about therapeutic aims, discharge processes, and home exercise programs
• Maintain accurate documentation of evaluations, treatments, and patient progress in compliance with regulatory standards
• Engage in collaborative care team rounds and interdisciplinary meetings for effective care coordination

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification preferred
• Specialized credentials (neuro, geriatric, etc.) are advantageous but not obligatory

Skills

• Prior clinical experience is highly desirable
