Role Overview The Care Facilitator is one of a patient's first contacts with our ChenMed family, serving as a patient advocate and delivering exceptional customer service. The incumbent checks patients in and out of the medical clinic, provides basic secretarial and administrative support, and commits to superior service. Responsibilities and Duties Interacts appropriately with patients and records information while greeting, checking‑in, checking‑out or scheduling appointments. Uses computer systems to enter new patient information, print relevant forms and schedule appointments. Ensures accurate documentation of patient contact and emergency contact information. Answers telephone calls, takes detailed messages, and directs them appropriately. Communicates information about ChenMed clinical personnel to patients and others. Verifies patient insurance via phone or website and collects necessary copays. Assists with scheduling follow‑up and in‑house specialist referrals when needed. Escorts patients to exam rooms, ensuring their comfort before they leave. Acts as a HEDIS champion for the PCP by providing visibility of HEDIS needs for scheduled patients. Provides a customer‑service survey at checkout and escalates issues for immediate recovery. Participates in daily and weekly huddles to share patient information. Other Responsibilities Maintains patient confidentiality of personal information and medical records. Reviews patients’ personal and appointment information for completeness and accuracy. Transmits correspondence by mail, email, or fax. Performs other duties as assigned and modified at manager’s discretion. Knowledge, Skills and Abilities Exceptional customer service skills and passion for serving others. Working knowledge of medical insurance and/or electronic medical record (EMR) systems. Knowledge of ChenMed medical products, services, standards, policies, and procedures. Skilled in operating phones, PCs, software, and other IT systems. Ability to communicate professionally and courteously with employees, patients, and others. Detail‑oriented; ensures accuracy of reports and data. Outstanding verbal and written communication skills. Strong listening skills. Positive and professional attitude. Proficiency in Microsoft Office Suite, including Excel, Word, and Outlook. Willingness to travel locally and regionally up to 10% of the time. Spoken and written fluency in English. Pay Range $16.5 – $23.56 per hour.
