Job Description

Provides nutrition assessment, counseling, education, and referral services to clients while evaluating nutritional needs, health risks, and eligibility requirements. Maintains accurate documentation, monitors program compliance and quality improvement efforts, and supports clinical audits and accreditation requirements. Collaborates with healthcare providers, clients, and community partners to coordinate services and ensure appropriate nutrition care and resources.

Minimum Requirements

• Bachelor’s or master’s degree in Nutritional Sciences, Community Nutrition, Clinical Nutrition, Dietetics, Public Health Nutrition, or Home Economics with emphasis in Nutrition. 15 hours of nutrition education required.
• Be over 18 years of age
• Possess a valid driver’s license
• Current CPR certification
• Prior experience preferred

Core Competencies

• Knowledge of electronic documentation program.
• Knowledge and ability to monitor and measure programs compliance with all applicable policies, funding and billing regulations and laws.
• Knowledge of compliance audits and ability to participate in audits related to documentation requirements, clinical quality, and program licensing regulations, and accurately report findings.
• Knowledge and ability to monitor all aspects of quality assurance/improvement and to implement plans to further improve quality.
• Knowledge and ability to assist in monitoring program’s compliance to applicable accreditation standards and implementing corrective action plans as necessary.

Job Duties / Skills Required

• Screens applicants for eligibility. This requires interviewing applicants to determine and verifying income for the household, and computing the information using electronic software for determination.
• Provides participant centered nutrition counseling and referral services for clients.
• Completes a nutrition assessment for applicant, utilizing the information to accurately assess nutrition risk and provide nutrition and health recommendations for the participant.
• Breastfeeding promotion for clients, community partners, and health providers. Refers breast-feeding clients to the Breast Feeding Peer Counselor Program.
• Prescribes Food packages tailored to the participant’s needs, issuing warrants following policies and procedures.
• Schedules and maintains appointments in computerized database.
• Refers high risk clients to Registered Dietician services for additional nutrition counseling.
• Knowledgeable and detailed operation of the Louisiana software for warrants preparation, distribution and record’s maintenance.
• When required, completes health measurements on all applicants. This includes height, weight, and blood samples. These measurements need to be completed following methods approved by the State of Louisiana Program as ensured by the Clinical supervisor.
• Provides clerical duties including the use of a copier, fax, computers and other office equipment, filing and web based operations and light housekeeping duties.
• Maintain a clear driving record in order to be able to drive company vehicles when necessary.
• Attends all staff meetings as scheduled.
• Other duties may be assigned.
