Licensed Practical Nurse (LPN) Near Concord, NH Overview: Our facility near Concord is seeking a Licensed Practical Nurse (LPN) to provide medical monitoring and care for clients undergoing withdrawal management. New graduates and experienced LPNs are both encouraged to apply. Position Offers Schedule: Monday, Tuesday, Friday, and Saturday Competitive pay: $38–46 per hour (DOE) Health, dental, and vision insurance Paid Time Off (PTO) Opportunity for growth Key Responsibilities Perform intake assessments, including gathering health histories and evaluating current symptoms and conditions Conduct COWS (Clinical Opiate Withdrawal Scale) and CIWA (Clinical Institute Withdrawal Assessment) withdrawal assessments Provide medical monitoring for clients undergoing withdrawal Administer medications safely and accurately, following facility protocols and provider orders Monitor and record vital signs, promptly reporting changes in patient status Support development of individualized care plans in collaboration with RNs and other healthcare professionals Document all patient interactions and interventions accurately in the EHR Collaborate with the interdisciplinary healthcare team, including participation in care conferences Qualifications Active, current Licensed Practical Nurse (LPN) licensure in New Hampshire Comfortable performing withdrawal assessments and medical monitoring Strong communication and patient education skills Why Join Us Join a facility where your nursing skills directly support clients through the withdrawal management process, with the opportunity to make a meaningful impact during a critical stage of recovery.
