Essential Functions:

• Evaluate residents' physical conditions and develop individualized treatment plans
• Provide therapeutic exercises and interventions to improve strength, balance, and mobility
• Collaborate with nursing staff, physicians, and other rehabilitation team members
• Document resident progress and adjust treatment plans as needed
• Educate residents and caregivers on techniques for continued progress

What We're Looking For:

• Current PA licensure as a Physical Therapist
• Degree from an accredited Physical Therapy program
• Experience in a skilled nursing or long-term care setting preferred
• Strong communication and teamwork skills
• Compassionate and resident-centered approach
