About the Role

As a Registered Nurse with Synergy HomeCare, you will play a vital role in developing, implementing, and overseeing client care plans. You will ensure each client receives safe, personalized, and high-quality care tailored to their needs supporting independence while prioritizing safety and dignity.

This role is ideal for an RN who enjoys autonomy, relationship-building, and guiding caregivers through clinical best practices.

Key Responsibilities

• Conduct in-home client assessments and reassessments
• Develop, implement, and update individualized care plans
• Provide clinical oversight and recommendations for non-medical and personal care services
• Educate and train caregivers on client-specific needs and best practices
• Communicate regularly with clients, families, and office staff
• Ensure compliance with state regulations and agency standards
• Support quality assurance and documentation requirements

Qualifications

• Active RN license (in good standing)
• Home care, geriatric, or community health experience preferred
• Strong assessment and documentation skills
• Excellent communication and interpersonal abilities
• Ability to work independently while collaborating with a supportive team
