Key Responsibilities: Coordinate Care: Facilitate warm hand-offs and referrals between patients, providers, and community services. Screen & Support: Conduct SDOH screenings (PRAPARE) and help patients set goals to overcome barriers. Educate & Advocate: Assist with applications for coverage, benefits, and other essential services. Document Thoroughly: Maintain detailed electronic patient records, tracking outcomes and service plans. Engage the Community: Participate in outreach events and consortiums to promote One Health services. Navigate Resources: Stay current on eligibility rules, insurance options, and enrollment procedures.
