Discharge Coordinator Full-Time
The Discharge Coordinator will work with parents, outpatient providers, and therapists to establish safe and supportive aftercare plans. Streamwood Behavioral Healthcare System (SBHS) is a 178-bed acute care facility located in Streamwood, IL and has been providing mental health treatment to the Chicagoland community and the state of Illinois since 1991. Qualifications
- Education/Experience: Bachelor's Degree required. 1 year experience in Case Management preferred. Experience and/or knowledge of mental health systems.
- Skills: Organizational skills, computer skills including excel, strong communication skills.
