Medical Social Worker - Care Coordination - FT Days

Job Summary The Medical Social Worker Unlicensed collaborates with the Clinical Care Coordinator (C3) and other members of the care management and health care teams, utilizes professional Social Work skills in accordance with social work practice standards to: assist the patient in achieving optimal health; assure appropriate use of health care resources; facilitate/coordinate timely care along the continuum; decrease unnecessary variances in care through proactive care management; develop a safe and timely discharge plan in accordance with the Joint Commission and CMS Guidelines, ensuring each patient will be ensured a planned program of continuing care which meets his/her discharge needs and to ensure a smooth hospital stay with a seamless transition to the next level of care following discharge from GHS. Responsibilities Intervention/Planning Coordination/Implementation Regularly cross‑trains health system wide to sustain proficiency across multiple Social Services functional areas. Assess each patient, identify discharge planning needs and initiate a discharge assessment. Additionally, address and document management of social determinants of health. Identify psychosocial needs of patients and families including social determinants of health. When possible, adhere to cultural, religious practices and make adjustments for any special impairment. Discuss the home situation with patient/family/caregiver as needed. Coordinate homecare as needed. Discuss with the team to coordinate the best plan and identify any barriers to discharge.
