Home Health Occupational Therapist – Key Responsibilities Assess and screen patients for daily living/work‑related skills, and develop measurable therapeutic retraining programs. Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the level of function. Confer with the patient’s physician and other health‑care team members to develop and revise the Plan of Care. Provide therapeutic treatment and instruction in accordance with physician orders to improve or restore strength, coordination, range‑of‑motion, and function, or teach compensation measures. Review and expand retraining programs consistent with behavioral goals and program guidelines. Consult with vocational team members to develop and implement vocationally oriented plans that match patient needs and capabilities. Report patient status and progress to the physician, rehabilitation staff, Clinical Manager, and family members. Participate in care‑coordination, discharge planning, and serve as a resource to other health‑care personnel to meet patient needs. Design community‑reintegration activities to assist the client in physical reconditioning and psychological adjustment, coordinating the plan with the interdisciplinary team. Recommend and design special adaptive equipment for clients to improve residential or working environments and enhance participation in the program and community. Design and train staff and family members to carry out retraining programs, including dressing, feeding, grooming, hygiene skills, and participate in team and family educational meetings. Supervise, instruct, and evaluate occupational therapy assistant and home‑health aide performance in implementing occupational therapy services. Accurately and thoroughly document patient care observations, interventions, and evaluations on the day services are rendered; ensure interim verbal orders from the physician are documented and implemented; submit evaluation, treatment plans, and discharge summary to the supervisor and care‑management staff. Qualifications Current and unrestricted OT licensure. Current CPR certification. Valid driver’s license, auto insurance, and reliable transportation. Minimum six months of occupational therapy experience. Home health experience. Compensation Pay per visit/unit: $54.00 – $76.00. Additional Information TB Statement: This role is patient‑facing and part of Humana’s tuberculosis screening program. Selected candidates will be required to be screened for TB. Driving Statement: This role is part of Humana’s driver safety program. Candidates must have a valid state driver’s license and maintain personal vehicle liability insurance covering the state minimum limits or $25,000 bodily injury per person / $25,000 bodily injury per event / $10,000 property damage, whichever is higher. Work Schedule Scheduled weekly hours: 1. Pay Range (Full Time) $85,400 – $117,500 per year (based on location and qualifications).
