The role involves performing care coordination and discharge planning within an acute care setting. Candidates must possess 2 years of acute care hospital case management experience. Experience with assessing patients and developing treatment plans is essential. Knowledge of drug/alcohol abuse treatment options and services for patients experiencing homelessness is valued. The candidate should be able to arrange durable medical equipment, skilled nursing facility admissions, and ambulance transport independently. Participation in family meetings and interdisciplinary support is expected. Proficiency in computerized documentation and Microsoft applications is required.
